Country Transitional Living Services, Inc. (NCTLS) is committed
to enabling individuals to reach their fullest potential. The residential
services provide individuals the opportunity to live in the community
in a home-like atmosphere. The residential services offer adults,
who are involved in the mental health system, supports to successfully
live and work in community settings. The rehabilitative services
offer assistance to people in order to achieve independence. The
supportive services are individualized to provide opportunities
for successful community integration. Individuals are supported
in pursuing their goals and are encouraged to enhance their skills
to enable them to live in the community. Individuals are assisted
in developing a natural support system to compliment the mental
health care offered by service providers. Involvement in NCTLS
residential programs is voluntary with emphasis placed on consumer
choice. All programs are certified by the Office of Mental Health
(OMH) and maintain high standards according to a Quality Assurance
Program operated by NCTLS. Adult programs are based on the following
essential services:

* Substance Abuse Services

All
programs are transitional in nature, providing a continuum of care
to assist individuals in becoming more self-sufficient. The continuum
of residential care ranges from 24-hour on-site staff supervision
to case management services for individuals living in the community.
The most appropriate level of staff support is based upon individual
need. The following is a brief description of the residential services
offered for adults:
Supervised
Community Residence: Group home setting with staff assistance present
on-site 24 hours per day. This setting offers the most intensive
level of care provided by trained staff. NCTLS has four community
residences located in Jefferson and St. Lawrence Counties.
Supportive
Apartments: Apartments with staff members who support participants
living in NCTLS leased apartments in Watertown, with face-to-face
visits dependent upon individual need. Participants typically live
with a roommate while continuing to work toward the goal of independent
living. Staff are available to residents on a 24-hour basis through
an on-call system.
