North
Country Transitional Living Services, Inc. (NCTLS) provides Case
Management Services for people with eating disorders in the North
Country. A coalition of local agencies provides comprehensive
seamless services to children and adults with eating disorders
from Jefferson, Lewis, Oswego, and St. Lawrence Counties in conjunction
with Unity Health System and Golisano Children’s Hospital
in Rochester. This coalition is part of the
Eating Disorders Recovery Center of Western New York.
On
a local level, collaboration will occur between NCTLS, families,
and other health care providers.
Need
for case management services is essential in our three county
area because of the distance from major metropolitan areas where
eating disorder services are available. The purpose of the case
management program is to eliminate fragmented and uncoordinated
service in this area.
The Case Manager will:
· Bridge gaps
· Coordinate clinical services
· Coordinate individualized plans for care
· Link patient and families to services and supports
· Intercede on behalf of the family to gain access to needed services
and supports in the least restrictive setting
· Monitor services and supports
· Maintain goal oriented progress notes
· Conduct service plan reviews
· Complete evaluation instruments as designated
· Ensure information and education regarding available services for eating
disorders is provided to families, schools, and other community based agencies
Referral
Individuals
may self-refer to Eating Disorders Services or be referred by
health care providers. Referrals can be made by calling the Eating
Disorders Case Manager at (315) 782-1777 Ext. 34. Intake appointments
are scheduled as quickly as possible. Primary care physicians
are also asked to confirm patient medical stability.
