Home and Community Based Services Waiver
HCBS Waiver provides in-home services for residents of Jefferson, Lewis, St. Lawrence and Herkimer counties. Children (5-18 years) who are at risk for out of home placement and meet other criteria are eligible for services. Parents of children in the program can receive support from a Parent Partner.
B2H WSP Provider
TLS partners with Hillside Children and Family Services to provide Waiver Services for the children and families enrolled in the Bridges to Health Waiver Program (B2H Waiver). B2H services are available to children in the Foster Care System. TLS is contracted to provide these services in Jefferson, Lewis, St. Lawrence, Herkimer, Onondaga, Oneida, and Oswego Counties.
